What's inside
What's inside
Refill countdown, computed from one date
Set Refill by on a Medication and Days left and a Status badge (๐ด Refill now, ๐ก Refill soon, ๐ข Stocked, ๐ต Ongoing for as-needed meds) update on their own as the date gets close.
Every person's meds and appointments, together
Open anyone in People to see just their own Medications and Appointments side by side โ the fastest way to check on one family member without scrolling past everyone else's.
Next appointment, per person and per doctor
Appointments roll up to both People and Doctors as a friendly 'in N days' or 'No upcoming appointments' line, always the soonest one still ahead.
Doctors track who they see and what they prescribe
Each Doctor rolls up Appointment count and Meds prescribed, so you can see their caseload with you and your family in one row.
Organized, not clinical
This template is for provider contacts, refill timing, and appointment scheduling โ it doesn't diagnose anything or offer medical advice.
A weekly health check-in
A built-in checklist points you at what needs a refill, what's coming up this week, and anyone whose Overview looks worth a second look.
How it works
How it works
Every Medication and Appointment links to one Person, and can also link to a Doctor. Refill by feeds a Days left countdown and a Status badge on each medication. Appointments roll up to both People and Doctors as a 'days until the soonest one' number, which turns into a friendly Next appointment line on each person's and each doctor's own page.

How to use
- 01
Every Medication and Appointment links to a Person โ open anyone in People to see just their own Medications and Appointments together, side by side.
- 02
Days left and Status on Medications are never typed by hand โ they're computed from Refill by, so a ๐ด Refill now or ๐ก Refill soon badge appears on its own as the date gets close.
- 03
No Refill by date? That's fine โ an as-needed medication (Frequency: As needed) just shows ๐ต Ongoing until you set one.
- 04
Doctors rolls up every appointment and prescription tied to them, so Next appointment always shows the soonest upcoming visit โ or a gentle "No upcoming appointments" when there isn't one yet. People shows the same digest per person, plus how many medications need a refill.
- 05
Pair this with the Mood & Wellness Journal to track how you're feeling alongside what you're taking.
- 06
The ๐ Add a medication and ๐ Add an appointment dashboard buttons can't set who it's for โ open Needs linking in Medications or Appointments to add the Person by hand.

ยง FAQ
FAQ
Does Notion have a medication tracker?
Not built in. Health Records & Medications is a Notion template that adds one: every Medication links to a Person, and Refill by feeds a Days left countdown and a Status badge that updates on its own.
Can Notion remind me when a prescription needs a refill?
Not with an automatic push notification. The Medications database computes a Status badge (๐ด Refill now, ๐ก Refill soon) you check on the dashboard's Needs refill card; for an actual alert, set your own reminder on that page using Notion's own reminder feature.
Does Notion have a health tracker for the whole family?
Yes. People is the hub โ add each family member, then link their Medications and Appointments to them. Open anyone's own page to see just their own two lists together.
Is this medical advice?
No. This template is organizational โ provider contacts, refill timing, and appointment scheduling. It doesn't diagnose anything or replace advice from an actual doctor or pharmacist.
Does it work on Notion's Free plan?
Yes โ every formula and rollup, including the Next appointment digest on People and Doctors, works on Notion's Free plan.
